Thank you for considering AK Rheumatology for your care Please note the following

 
New patient Intake forms that are sent via text and required to be completed prior to the visit.
  1. We request a referral from your PCP including the last note, labs or any imaging. Any previous rheumatology notes are also appreciated.
  2. We do not take any Medicaid plans, Molina, apple health.
  3. We do take most commercial insurances and Medicare. Please call to confirm.
  4. We charge a no-show fee for any cancellations within 24 hours of appointment or Missed appointments.
Please call us 24 hours prior to your appointment if you can not make to the appointment.

We can offer Telehealth services to residents of Washington, Oregon and California Telehealth service to California residents will be ONLY cash pay.

  • New patient visit - $300
  • Follow up visit - $250
We can offer a Payment Plan.
